Office for Mental Health and Wellbeing, ACT Health works with all ACT directorates to cover the whole of the Australian Capital Territory. The Office for Mental Health and Wellbeing implements a whole of government and whole of community approach to achieve the territory wide vision for mental health of : “A kind, connected and informed community working together to promote and protect the mental health and wellbeing of all”.
Healthy North Coast PHN
The Healthy North Coast PHN is bordered on the east by the coast, and to the west the New England Tablelands escarpment and a line of state forests and national parks. The PHN’s objectives focus on supporting North Coast communities to achieve their best health outcomes. This is sought to be achieved through a range of key initiatives, empowering consumers, clinicians and other service partners to work together to develop a more integrated health and social care system which acknowledges that people’s best lives are dependent on multiple factors.
Brisbane South PHN
The Brisbane South PHN region covers a large area south of the Brisbane River, includes metropolitan, rural and remote island locations. Our region is home to approximately 23% of the Queensland population - over 1.2 million people. Brisbane South is home to many different backgrounds and cultures including the second-largest urban Aboriginal and Torres Strait Islander population. We are the region of highest refugee settlement in Queensland, and 30% of Queensland’s population who were born overseas live in our region.
Western Sydney PHN, operated by Wentwest
The Western Sydney Primary Health Network, operated by WentWest, is one of NSW’s fastest growing areas covering a region that encompasses four Local Government Areas including Blacktown, The Hills Shire, Cumberland, and Parramatta. WentWest, the Western Sydney PHN’s mission is to work in partnership to lead better system integration and coordination, strengthening equity and empowerment for Western Sydney communities.
Nepean Blue Mountains PHN
The Nepean Blue Mountains PHN is provided by Wentworth Healthcare whose mission is to empower general practice and other healthcare professionals to deliver high quality, accessible and integrated primary healthcare that meets the needs of people in the Blue Mountains, Hawkesbury, Lithgow and Penrith regions.

The Western NSW PHN covers Broken Hill to Oberon to Lightening Ridge to everyone in-between, making it the largest PHN in NSW. Their vision is to aim at supporting, strengthening and shaping a world class person-centred primary health care system in Western NSW.

Greater Southern is one of eight regions serviced by WA Primary Health Alliance, Country WA PHN. As WA’s most densely populated country region, the local team is helping people at risk of poor health outcomes to access high quality care close to home, with a particular focus on end of life care.
Pilbara - Country WA PHN
Pilbara is one of eight regions serviced by WA Primary Health Alliance, Country WA PHN. As one of Western Australia’s most remote regions and with a significant Aboriginal population, the local team is focused on joining up the region’s health system to improve access to care and outcomes.
